Sport's Physical Examination Night
On Tuesday, April 7th a physical examination night will be held at Logansport High School.
The cost is $15.00. This is available to all 7th & 8th grade students who wish to participate in extracurricular sports next year.
You can prepay for your physical in the Logansport Junior High School office. Individuals who prepay will be seen from 3:15pm-4:30pm on the 7th. If you are prepaying, your money must be turned in to the office by 9:00am, April 7th.
If you do not prepay, you can pay at the door on April 7. Individuals who do not prepay will be seen from 4:30pm-5:45pm.
Physical forms are available in the Junior High School office.
8th grade students going to the high school, will use the green form.
7th grade students going into 8th grade will use the cream-colored form.
The front page of the form MUST BE filled out and signed by your parent before arriving at the high school.
This is a great deal, and the physical is good through May of 2021.
